Community Outreach Service, Enfield

The Charity's community outreach support service provides adults with mental health needs a personalised care package based on individual reqirements.

Introduction
The service has been awarded an overall Care Quality Commission rating of three stars, which means the people who use this service experience excellent quality outcomes.

Support may include:
Encouraging individuals to actively prepare meals and look after their home appropriately, highlighting any health and safety issues.

Helping people to apply for benefit entitlements and manage their finances to pay for household bills, rent and shopping.

Encouraging individuals to link in with and access their local community resources, support to promote social inclusion, self-determination and the confidence to engage in a rewarding activity such as a part time college course or voluntary work.

Supporting individuals to maintain their physical and mental helath, to manage independently and keep medical and other necessary appointments.

Aim
The aim of this service is to help individuals attain personal goals, achieve desired outcomes and realise thier potential.
