Supported Living Services
The Charity's supported living services in Hertfordshire provide people with a home of their own and the support they require to sustain it. This service is also able to offer an outreach package in the person's own home, through their care package.
Introduction
The service is for adults who have a learning disability (and with/without a mental health problem), are in housing need or are inappropriately housed and have a local connection with the Welyn Hatfield area.
Support
The support package is tailored to meet the individual's needs. This is identified through our referral and assessment process.
Support includes:
Encouraging individuals to actively prepare meals and to look after their home appropriately, ensuring health and safety requirements are met, and any faults and repairs are reported immediately.
Helping people to apply for benefit entitlements and manage their finances to pay households bills and rent.
Encouraging individuals to link in with their local community resources, support to promote social inclusion and the confidence to engage in a rewarding or fulfilling activity such as attending a college course or voluntary work.
Encouraging individuals to maintain their physical and mental health, manage their medication independently and to keep medical and other necessary appointments.
Properties include a 24-hour link to the local Community Alarm Team who can contact a member of staff on-call for support with medical or housing emergencies.
Cost
The supported living services are currently funded by the Supporting People funding. People who use the service asre supported to apply for the necessary housing related benefits and need to make individual rent contributions.
Support Team
Committed and appropriately trained staff and Brothers of the Hospitaller Order of Saint John of God.
